This track examines organization and delivery of long-term care for individuals with significant functional limitations and complex needs. Topics include residential care, home- and community-based services, integrated care pathways, and case management approaches. Long-Term Care Models and Integrated Care Systems Presentations will evaluate models that coordinate medical, nursing, rehabilitative, and social support across settings. The session will address quality indicators, person-centered planning, workforce training, and family caregiver involvement. Comparative analyses of long-term care systems, financing mechanisms, and reforms in different countries are welcome. The focus is on evidence-based strategies that enhance continuity, safety, and outcomes for highly dependent older adults.
