Geriatric Medicine and Multimorbidity Management
This Geriatric Medicine and Multimorbidity Management session focuses on comprehensive care for older adults with multimorbidity, polypharmacy, and complex functional needs. Topics include geriatric syndromes, comprehensive geriatric assessment, care planning, and optimization of medication regimens. Presentations will explore models of interdisciplinary care, integration of geriatric principles into primary and specialty care, and transitions between care settings. The track will highlight evidence-based strategies to manage multiple chronic conditions simultaneously, prioritizing patient-centered outcomes such as function, independence, and quality of life. Research on prognostication, risk stratification, and decision aids for high-stakes clinical choices in multimorbid older adults is particularly welcome.
